As part of SPARK's collaboration with the PCNs and Hertfordshire Mind Network, we are offering:
Up to six sessions either virtually via Zoom or face-to-face, that might incorporate Care Navigation, Health and Wellbeing Coaching, Social Prescribing or other modalities;
Availability of after-school appointments to ensure that our service is accessible;
A weekly drop-in service for 10-18 year olds.
All without a waiting list! Our partnership is about our values of providing effective support, fast. We were also excited to be a part of a progressive provider collaborative that would transform access to mental health and wellbeing support for children, young people and families in Ware and the surrounding areas.
Hailey View Surgery is a part of Hoddesdon and Broxbourne Primary Care Network, not Hertford and Rurals Primary Care Network. H&B PCN are not one of our partners at this time.
If you are seeking a referral through one of the GP practices above: Simply ask for a referral to SPARK following an appointment with your GP or another member of the practice team. Failing that, fill in one of the forms below:
As part of our partnership with Hertfordshire Mind Network, we are able to offer a drop-in mental health service which is available regardless of which GP surgery they are registered with. The only criteria is:
They need to be no younger than 10 and no older than 18; and
They need to be resident within the county of Hertfordshire.
That's it! No referral, no appointment needed. The service operates on Wednesdays from 3:30PM until 6:30PM with the last arrival being at 6PM. The address is below:
